Six Steps for Forest Grown Verification

Thank you for your interest in applying to the Forest Grown Verified program!

The Forest Grown Verification Process

Download and fill out the applicable document(s) below. Submit them to tanner@unitedplantsavers.org to complete your application.

Once your application has been received, we will contact you to schedule an interview. *Please note that due to COVID-related concerns with travel all interviews will be conducted “virtually” for the remainder of 2020.

We have grant funding to cover all application fees for 2020. Folks are encouraged to take advantage of this limited opportunity to enroll. Contact tanner@unitedplantsavers.org for details.

Application Fee: $200
Covers initial verification process

Renewal Fees – Due each year with annual report to maintain status
For Producers/Harvesters
• Each harvest year: $200
• Each non-harvest year: $50
For Processors/Handlers
• Annual renewal fee: $200

Forest Grown Documents

  • Program Manual
    FGV Program Manual– Download and read the manual. Don’t hesitate to ask for clarification on any aspect of the program.
  • Application Forms
    • For Growers and Processors
      FGV System Plan – If you are a grower you need to fill out the Forest Grown Verified System Plan.
      FGV Land Use Affidavit – You may need the Land Use Affidavit if you are leasing land for your operation.
    • For Sellers & Producers
      FGV Product Profile – If you are selling a product or producing a product fill out and submit the Forest Grown Verified Product Profile to apply.
  • Submit Form(s) & Arrange Inspection
    FGV Inspection Report – This will be filled out by the inspector.
  • Acceptance
    After an inspection has been conducted, you will be notified upon acceptance and awarded a certificate of verification will be provided for your records.
  • Annual Report
    FGV Annual Update – Participants in the program must file a report each year to maintain their status. This report will be due one year from the date of acceptance into the program.
